'To Err Is Human'
In 1999 the IOM shook the pillars of medicine with a report saying as many as 98,000 people die every year in the United States from preventable medical error. The report is today still the subject of much discussion and many follow-up studies.

As a follow-up to the groundbreaking 1999 report, in 2001 the IOM released "Crossing the Quality Chasm," stating quite clearly that the gap between the healthcare Americans have and could have is broad and deep.
